Any further information that is gathered as part of your assessment, diagnosis and treatment is seen only by the psychologist conducting your treatment. This information is retained in order to document what happens during your sessions to enable the psychologist to provide a relevant and informed psychological service.

Disclosure of personal information

All personal information gathered by Seed Psychology during the provision of the psychological service will remain confidential and secure except where:

  1. It is subpoenaed by a court, or
  2. Failure to disclose the information would place you or another person at serious imminent risk; or
  3. Your prior approval has been obtained to:
    1. Provide a written report to another professional or agency (e.g. GP). Please note if you have been referred under Medicare, reports at 6 and 10 sessions are provided to the referrer.
    2. Discuss the material with another person (e.g. employer, spouse)
    3. If disclosure is otherwise required or authorised by law

Your psychologist has a legal and ethical responsibility to keep all psychological services conducted via face to face or telehealth, confidential unless under circumstances previously specified.
